Veloxis Pharmaceuticals, Inc. (“Veloxis” or the “Company”) has established an ethics and compliance program that reflects our commitment to the Veloxis mission, core values, and to conducting business in compliance with the laws, regulations, guidance, and industry standards that govern our organization.

Ethics and Compliance Helpline

Veloxis is committed to essential principles of good corporate conduct and integrity across all its activities. If Veloxis becomes aware of violations of law or Veloxis policy, we will investigate the matter and, where appropriate, take disciplinary action and implement corrective measures to prevent future violations.

We have set up an external compliance helpline system called the Veloxis Anonymous Compliance Reporting Helpline—operated by an independent, certified third-party vendor for our employees and related parties to report potential violations of laws or Company guidelines.

Code of Conduct

Veloxis and the Open Payments/Sunshine Act

We are committed to transparent and accurate reporting under the Open Payments/Sunshine Act, which requires industry-wide reporting of certain payments made to health care providers and teaching hospitals. Examples of payments Veloxis reports under the Open Payments/Sunshine Act include: consulting fees and compensation for services such as speaking engagements; meals, travel, and lodging costs associated with educational programs, advisory boards, or other business meetings; charitable contributions; research funding and grants; and education-related items and other lawful courtesies.

How to Search for Payment Information Using the CMS Open Payments Search Tool

  1. Visit the CMS Open Payments searchable database website at https://openpaymentsdata.cms.gov/. 
  2. Use "Search Open Payments Data" or "Advanced Search" to search by:
    1. Reporting manufacturer (e.g., Veloxis);
    2. Provider or teaching hospital; and/or
    3. Geographic location, specialty, or other available filters.
  3. Select the Covered Recipient or Manufacturer generated by the search results, which will display, by year, the itemized payment records, including the type of payment or transfer of value, the amount, the date, and the reporting manufacturer.

In accordance with the Colorado Wholesale Acquisition Cost Disclosure Law (Colorado Revised Statutes Section 12-42.5-308), Veloxis is disclosing to authorized prescribers of prescription drugs in Colorado the wholesale acquisition cost (WAC) pricing information of ENVARSUS XR® (tacrolimus extended-release tablets).

In accordance with Connecticut Public Act No. 23-171, Veloxis is disclosing to prescribing practitioners and pharmacists in Connecticut certain required wholesale acquisition cost (WAC) pricing information of ENVARSUS XR® (tacrolimus extended-release tablets).

Vermont

In accordance with Vermont Pharmaceutical Marketer Price Disclosure Law 18 V.S.A. Section 4633, Veloxis is disclosing to authorized prescribers of prescription drugs in Vermont certain required average wholesale price (AWP) pricing information of ENVARSUS XR® (tacrolimus extended-release tablets) Long Form and Short Form
